This Turkish-type, polygon tip rhinoplasty was achieved using all internal incisions and leaving the dorsum intact - in a pushdown reduction.

This patient wanted a slightly narrower nose and finer tip. She did not want to lose her natural beauty - simply wanted to enhance it. With a small tip graft and weir excisions (nostril reduction), she has less flaring of the nose and a stronger tip.

When the dorsum is flat or under-projected, there is a completely different approach to rhinoplasty. This patient wanted a pretty, “button” type nose. After an precision open rhinoplasty, she has a beautiful concavity, or slope, to her dorsum and a soft, pert tip.

Desiring a smaller more feminine nose, this patient underwent a closed (no columella incision), dorsal preservation rhinoplasty. Because her tip height was reduced to take down the overall size of her nose, the nostrils need to be reduced, as well. Think of the flaps of a tent - when the central pole is shorter the tent flaps become floppy and redundant. Similarly, when the nasal tip height is deprojected (made shorter), the nostrils can look wider in appearance. This is why we removed a small amount of nostril tissue (weir excisions) - done at the base of the ala.

Using traditional rhinoplasty techniques and a precision reduction of her dorsal hump, this patient has a beautiful and stable longterm result. She look very natural - she specifically requested to not have an “operated-look”. She looks elegant and is breathing well after the repair of her deviated septum.
